The Opportunity
As our Community Health Nurse, you will work closely with the Health Lead to provide culturally safe nursing care across the lifespan; from newborns to Elders. You’ll deliver hands-on clinical services, provide health education, support chronic disease management, and help promote overall wellness throughout the community.

Job Summary
We are seeking a compassionate and dedicated Community Nurse to provide essential healthcare services within the community. The ideal candidate will deliver patient-centered care, promote health and wellness, and support individuals in managing their health conditions. This role offers an opportunity to make a meaningful difference in patients' lives through personalized nursing care in diverse settings.
Responsibilities
- Conduct home visits and community-based assessments to evaluate patients' health status
- Educate patients and their families on health management, disease prevention, and wellness strategies
- Monitor patient progress and document care activities accurately in EMR
- Collaborate with multidisciplinary teams to coordinate care plans and ensure continuity of care
- Assist with health screenings, immunizations, and health promotion programs within the community
- Respond to patient inquiries and provide guidance on managing chronic conditions or acute illnesses
Skills
- Proven experience in patient care and nursing practices
- Strong understanding of medical terminology and clinical procedures
- Excellent communication skills to effectively educate and support patients and families
- Ability to work independently while maintaining a high standard of professionalism
- Compassionate attitude with a genuine desire to improve community health outcomes
- Organizational skills to manage multiple patient visits efficiently
