Tsi Kanonhkhwatsheríyo Indigenous Interprofessional Primary Care Team (IIPCT) provides wholistic, culturally grounded primary care across three sites, with a fourth location opening soon.
We are a team of physicians, nurses, social workers, community health workers, traditional care workers, and other allied health professionals working together to provide integrated, team-based care for Indigenous individuals and families across our region.

JOB SUMMARY:
We are seeking a Registered Physiotherapist to join our innovative, interdisciplinary primary care team. This role supports patients across the lifespan, with a focus on function, mobility, chronic disease management, and prevention within a primary care setting. As part of a collaborative team, the Physiotherapist will play a key role in improving access to musculoskeletal care, reducing pain and disability, and supporting patients in maintaining independence and quality of life.
EDUCATION, EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS:
- Degree in Physiotherapy from an accredited university
- Current registration in good standing with the College of Physiotherapists of Ontario
- Professional liability insurance
- Membership with the Canadian Physiotherapy Association is an asset
- Experience providing physiotherapy care with a focus on musculoskeletal and orthopedic conditions
- Additional experience in pediatrics, pelvic health, pre/postnatal care, oncology rehab, or chronic disease management is an asset
- Strong assessment, clinical reasoning, and treatment planning skills
- Ability to provide patient-centered education and self-management support
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
- Ability to work both independently and within a team-based care model
- Commitment to culturally safe, trauma-informed care
- Experience in community or primary care settings is an asset; new graduates are welcome to apply
KEY RESPONSIBILITIES:
- Assess and treat patients referred to physiotherapy within a primary care context
- Develop individualized, goal-oriented treatment plans in collaboration with patients and the care team
- Provide interventions including exercise prescription, manual therapy, and functional rehabilitation
- Support prevention and chronic disease management through education and self-management strategies
- Collaborate with physicians, nurses, and allied health professionals to provide coordinated, team-based care; participate in case conferences and shared care planning
- Document care in a shared EMR system
- Support continuity of care through communication with community partners and services
- Participate in quality improvement initiatives and program development
- Provide supervision to physiotherapy assistants, students, or other support staff as needed
- Deliver care in a manner that is respectful, culturally safe, and responsive to community needs
