Key Responsibilities:
- Conduct thorough evaluations of geriatric patients, considering their medical history, psychological status, and functional capabilities
- Develop and implement individualized care plans tailored to the unique health challenges faced by older adults
- Manage chronic illnesses and coordinate care across various medical specialties as needed
- Provide education and support to patients and their families regarding health conditions, medications, and lifestyle changes
- Collaborate with a multidisciplinary healthcare team to optimize patient outcomes and enhance overall well-being
- Maintain accurate and compliant medical records and documentation
- Engage in continuous professional development to stay informed on the latest practices and treatments in geriatric medicine
Requirements
- MD or DO degree from an accredited institution
- Board certification in Geriatric Medicine is essential
- Active medical license in the relevant state
- A minimum of 3-5 years of experience in geriatric medicine or related fields
- Extensive knowledge of age-related medical conditions and their management
- Exceptional communication and interpersonal skills, with a compassionate approach to patient care
