We are looking for a pharmacist to join our Pharmacy Services team!

Are you ready to advance your pharmacy career in a dynamic hospital environment? We are seeking motivated pharmacists to join our team of pharmacists, supporting both daytime clinical services and overnight medication management.
Role Overview
This position offers a blend of daytime clinical pharmacist responsibilities with daytime, evening and overnight order verification and drug distribution activities to support our 24/7 pharmacy operations. You will rotate between daytime clinical shifts, daytime, evening and overnight distribution shifts, ensuring comprehensive patient care and operational excellence using our fully electronic medical record.
Daytime Clinical Pharmacist Shift Activities
- Identify, prevent, and resolve drug therapy problems
- Develop and document therapeutic plans
- Optimize medication selection, dosing, and monitoring
- Collaborate within a multidisciplinary team
- Participate in hospital and departmental projects
- Teach and mentor learners, pharmacists, and students
Daytime, Evening And Overnight Distribution Shift Activities
- Execute inpatient medication management across Charlton, West 5th, and other hospital locations requiring offsite support based on the site hours of operation
- Conduct clinical review and verification of medication orders for safety and appropriateness
- Provide drug information and clinical support
- Oversee drug distribution services in partnership with Pharmacy Technicians
- Communicate effectively with nursing, medical staff, pharmacy team members, and management

Qualifications
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university required
- Registration with the Ontario College of Pharmacists, Part A, in good standing
- Previous hospital experience is an asset
- Hospital Pharmacy Residency preferred
- Injection and Immunization Certification from an OCP-recognized program preferred
- Certification or advanced training in disease state management is an asset
- Demonstrates strong customer service orientation and skill working collaboratively with others
- Experience working on or leading quality improvement initiatives is an asset
- Possesses excellent oral and written communication skills
- Excellent attendance and punctuality are essential
Responsibilities
- Review medication orders for safety and appropriateness
- Provides customer centric care, recognizing both patients and other internal stakeholders (eg: other healthcare providers) as customers
- Provides pharmaceutical care to patients while working under full expanded scope of practice
- Ensures optimal patient care by identifying, preventing and resolving drug therapy problems
- Oversee tasks performed by Pharmacy Technicians to ensure safe and accurate medication dispensing
- Precepts and mentors students and residents
- Attend and present at staff education seminars
- Performs duties in a manner that demonstrates the employee’s commitment to developing a culture of patient and staff safety through accountability, reliability, trust and team work.
- Uses professional communication and excellent interpersonal skills to work effectively with colleagues within and outside the department
- Effectively manages daily workload using demonstrated ability to prioritize patients and tasks
- Identify and follow up on adverse reactions and medication incidents
- Contributes to the corporate efforts and initiatives, as appropriate, to enhance SJHH’s mission, vision and values, by participating in departmental and hospital projects, committees and task groups as assigned
- Acts as a liaison between Pharmacy Services and other healthcare provides (eg: nurses, allied health staff and physicians) to provide education and to promote pharmacy initiatives
- Maintains confidentiality and privacy standards at all times
- Adheres to all applicable NAPRA/OCP regulations and hospital/Pharmacy Services policies and procedures
- Performs all duties as assigned by Manager
NOTE: This position will be mainly at Charlton Campus, but may be scheduled to work at other campuses as well.
