Summary:
Goldbelt Glacier is looking for a Clinical Nurse Behavioral Health Care Facilitator.

Responsibilities
Essential Job Functions:
- Practice independently and collaboratively with other members of the health care team to ensure a comprehensive plan of care for the patient.
- Provide educational lectures and participate in in-service training to staff members.
- Perform developmental assessments and screenings; conducting diagnostic and screening tests; initiating and evaluating treatment regimens that may include prescribing and dispensing medication appropriate to the privileged scope of care.
- Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
- Recognize conditions that require isolation and ensure universal precautions are used in all patient encounters.
- Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement.
- Maintain a level of productivity comparable with that of other individuals performing similar services.
- Comply with the standards of The Joint Commission, applicable provisions of law, and the rules and regulations of any and all governmental authorities pertaining to licensure and regulation of health care personnel and medical treatment facilities, the regulations, and standards of medical practice and the MTF’s medical staff bylaws.
- Adhere to and comply with the Department of the Navy, and local instructions and notices in effect during the term of the contract.
- Participate in peer review and performance improvement activities.
- The HCW shall comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility.
- Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and MTF policy/instruction, as applicable.
Qualifications
Necessary Skills and Knowledge:
- Ability to read, write, speak, and understand the English language fluently and maintain good communication skills with patients and other health care personnel
- Shall be physically capable of standing and/or sitting for extended periods of time and capable of performing all services required under the task order
- Has basic knowledge of psychopharmacological agents (e.g., can name commonly used anxiety and antidepressant medication, appropriate dose and first line recommendations for a specific symptom presentation)
- Adhere to and comply with the Department of the Navy, Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.
- Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook
- Ability to effectively apply core case management functions: a) assessment and identification of patients for case management and need for an established plan of care; b) collaboration with the patient, family/caregiver, and members of the health care team for developing an effective plan of care; c) facilitation of care coordination and communication among all involved parties; d) advocate for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.
- Ability to effectively apply core disease management principles to mental health conditions: a) assessment and identification of target populations; b) implementation and execution of Clinical Practice Guidelines or standards; c) collaboration and coordination of activities with other healthcare members; d) promotion and education of patient self-management actions; e) collection, analysis and reporting on identified process and outcome metrics.
- Knowledge of medical privacy and confidentiality Health Insurance Portability and Accountability Act (HIPAA), and accreditation standards of AAAHC and The Joint Commission (TJC).
- Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.
- Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations.
- Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
- The contractor shall refrain from the use of profanity, sexual or racial jokes, or other behavior that would normally be considered unprofessional in a medical setting.
Minimum Qualifications:
- Graduate with a Bachelor of Science in Nursing (BSN) degree from a nursing program accredited by Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
- Certified in the specialty required as a Clinical Nurse by the American Nurses Credentialing Center (ANCC) or the recognized national nursing certification organization for the specialty.
- Shall have a minimum of 3 years of experience in mental health nursing.
- Must have practiced a minimum of 12 months in a mental health setting within the last 36
- Maintains an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.
- Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Air Force, and DoD on-line training as required.
- Each HCW shall possess and maintain current certification in either the American Heart Association BLS for Healthcare Providers or American Heart Association Healthcare Provider Course.
- HCWs who do not hold current certification must acquire certification prior to initiating contract performance.
- Web based classes do not meet these standards.
- Other certification may be acceptable as specified in the task order.
- Certification card must display the American Heart Association or MTN emblem.
- The contractor shall provide a list to the COR of each HCW’s BLS status by the 10th of each month.
- Shall be current with and have completed all continuing education requirements specified by their professional licensure or certification.
- Must demonstrate clinical competency within the past two years in the clinical discipline required.
- Shall be in good standing and under no sanction or suspension by the Federal Government
- All HCWs will be required to access DHA information technology networks/systems containing sensitive information.
- Only HCWs who are U.S. citizens can be granted access to DHA networks/systems and sensitive information.
- HCWs shall represent an acceptable malpractice risk to the government.
- HCWs shall be in good standing, and under no restrictions, with the state licensure board in any state in which a license is held or has been held within the last 10 years.
- HCWs shall be current with and have completed all continuing education requirements specified by their professional licensure or certification.
- Ability to pass all background/suitability checks.
Preferred Qualifications:
- A thorough understanding of the military lifestyle and experience working with military family members
- Working knowledge of relevant military, state, federal, and local laws, and resources
- Highly developed written, oral, and presentation communication skills
- An active Security Clearance is preferred.
