Job description
The post holder is responsible for delivering a high standard of care to all residents, coordinating the overall care on the Unit in the absence of a more senior member of staff. Facilitating the education and support of unqualified staff and participating in the development of nursing practice in Blackrock Abbey Nursing Home.
Key Areas of Responsibility
Clinical:
- Assess, plan, implement and evaluate care for residents in a way that reflects the homes philosophy.
- Ensure a high standard of written documentation is maintained.
- Promote an effective approach to care by ensuring effective communication is maintained between all staff, residents and their families.
- Ensure that information is imparted to residents and their families in a manner which maintains an individual's privacy and dignity.
- Liaise with voluntary and statutory agencies to ensure continuity of care from the Home to the community, or continuing care setting when appropriate.
- Liaise appropriately, making referrals when required, with the multidisciplinary team within the community.
- Safely set up and maintain S.C. syringe drivers and administer all medications in accordance with home and HSE guidelines.
- Demonstrate evidence-based nursing care within the clinical area.
- Be competent in the venepuncture procedures relating to all blood tests.
- Ensure adequate precautions are taken to minimize the risk of cross infection.
- Work as part of the multidisciplinary team in supporting residents and their carers always and during the delivery of bad news, caring for the terminally ill residents and in the event of a death.
- Report to a senior nurse and ADON and where applicable Person in Charge (Director of Nursing) on the resident's condition both verbally and in writing where appropriate.
- Act in accordance with the Homes Manual Handling Policy, minimizing risks to residents and staff.
- Recognize, react to and report emergencies.
- Be able to work under indirect supervision
- Be responsible, in conjunction with the Unit senior nurse for maintaining a safe environment for residents, visitors and other staff in the home.
- Comply with, promote and oversee Infection prevention and control practices and procedures within the home
- Promote, comply with and oversee Safety and Fire safety practices within the home and fire and emergency procedures
- Report any risks identified within the home
- Establish and maintain relationships with residents based on the following: Respect, Equality and Promoting Resident’s Independence.
Managerial:
- Co-ordinate the care on the unit in the absence of the CNM.
- Participate in the development of policies and protocols.
- Ensure safety of Residents, Careers, Colleagues and the Homes premises through the Blackrock Abbey Nursing Home Health and Safety Policy and Procedures.
- Ensure that all clinical incidents or near misses are reported and the Homes reporting policy is followed.
Education:
- Work towards becoming a mentor for Carers training in Care Support through the FETAC course and support junior colleagues.
- Contribute to the education program within the unit.
- Assist in ensuring residents' and families' educational and information needs are met.
- Ensure one is educationally and professionally updated and meet the criteria of the HSE and An Bord Altranis standards.
- Participate in clinical audit programs as directed by Senior members of staff.
