Position Description
Position Summary
To provide collaborative, evidence-based patient care for the purpose of achieving improved medication-related therapeutic outcomes and continuity of care. To be the medication and pharmacy practice expert serving as a resource within the pharmacy department, inter-disciplinary teams and throughout the organization and share this expertise with the team by teaching and mentoring pharmacy learners, pharmacy team members and interprofessional team colleagues.
Working collaboratively within a multidisciplinary health care organization, the Pharmacotherapy Specialist provides medication use, clinical and pharmacy practice expertise as an integral member of the Pharmacy department and organization. The Pharmacotherapy Specialist provides expertise in delivering pharmaceutical care to patients, including verifying medication orders, assessing patients’ medication therapy and identifying and resolving actual and potential Drug Therapy Problems (DTPs). Pharmacotherapy Specialists act as a liaison between Pharmacy Services and other healthcare providers (e.g., nurses, therapeutics staff, and physicians) to provide education on medications and the medication use system, and to support pharmacy and organizational initiatives. The Pharmacotherapy Specialist also participates in medication-related committee work and projects within the Pharmacy department and organization. The Pharmacotherapy Specialist also responds to safety risks, identifies and takes an active role in quality improvement initiatives to improve patient safety. The Pharmacotherapy Specialist provides education and mentoring to other pharmacy team members, including pharmacists, pharmacy students, and residents. Pharmacotherapy Specialists also advise and provide drug information and medication-related education to other healthcare providers, patients, and their caregivers. Work is conducted within the scope and standards of practice outlined in the NAPRA Model Standards of Practice for Canadian Pharmacists, by the Ontario College of Pharmacists (OCP), and by hospital policies and procedures.
Qualifications & Skills
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university
- Registration with the Ontario College of Pharmacists, Part A, in good standing.
- Pharmacy Residency ACPR (or equivalent) designation is preferred.
- Prior experience working in a Canadian hospital as a clinical pharmacist is required.
- Certification or advanced training in disease state management such as Diabetes, Mental Health, Nephrology, Critical Care, Geriatrics, or other clinical area is an asset.
- Demonstrates strong customer service orientation and skill working collaboratively with others in a multi-disciplinary team environment.
- Possesses excellent oral and written communication skills.
- Must have excellent interpersonal skills.
- Excellent attendance and punctuality are essential.
- Demonstrated leadership qualities including ability to lead quality and change initiatives.
- Effective organizational, prioritization, and time-management skills.
- Ability to prioritize perform multiples tasks and work under pressure.
- Ability to work in a fast-paced, constantly changing environment.
- Demonstrates ability to work independently, including effective problem-solving and critical thinking skills.
- Demonstrates clinical and pharmacy practice expertise and incorporates evidence-based medicine in practice area.
- Proficiency in Microsoft 365, Outlook and EPIC is an asset
Responsibilities
- Reviews and adheres to all applicable Pharmacy Services policies and procedures
- Maintains Pharmacist Part A licensure with OCP.
- Communicates effectively with patients, their agents, and members of the healthcare team.
- Documents and maintains records in accordance with OCP and hospital/departmental requirements for both dispensing records and patient care plans.
- Utilizes full scope of practice as outlined by OCP, and applicable provincial/federal legislation, in alignment with hospital policies and procedures.
- Upholds NAPRA Standards of Practice, provincial/federal legislative requirements, OCP standards and Accreditation Canada standards when delivering pharmaceutical care.
- Demonstrates clinical and pharmacy practice expertise pertaining to the portfolio responsibilities and pharmacy practice.
- Continuously develops one’s own knowledge and skills in applicable practice areas and demonstrates a commitment to continued professional growth by engaging in a lifelong learning process
- Performs cross functional duties as assigned by manager including providing clinical pharmacy services for patient care areas or alternate portfolio responsibilities when assigned.
