Mid-Main Community Health Centre is seeking a Registered Nurse to join our collaborative primary care team.
About the Role
As a Registered Nurse (RN) at Mid-Main, you will play a key role in providing comprehensive primary care, with a strong focus on chronic disease management, health promotion, and continuity of care across the lifespan.
You’ll work closely with physicians, nurse practitioners, and administrative staff to support patients in maintaining their health over time—whether that’s helping families navigate care, supporting seniors to stay well and independent, or guiding patients in managing ongoing medical conditions.
You’ll be an integral member of an interdisciplinary team, collaborating daily to ensure coordinated, high-quality care and a seamless patient experience.
This role reflects the full scope of community-based primary care, including preventive care, episodic needs, and coordinated support for patients experiencing mental health and social complexities.
What You’ll Do
- Collaborate with an interdisciplinary team to deliver integrated, patient-centered primary care
- Support chronic disease management (e.g., diabetes, hypertension) through monitoring, education, and care planning
- Provide care across the lifespan, including families, adults, and seniors
- Promote health and prevention, including lifestyle counseling, screening, and patient education (1:1 and group)
- Provide direct clinical care within your RN scope of practice
- Develop and coordinate care plans tailored to patient needs in a team-based model
- Conduct physical and mental health assessments, identifying and addressing changes in health status
- Support patients with mental health and social challenges through coordinated, team-based care
- Build strong therapeutic relationships with patients and families
- Act as a clinical resource and contribute nursing expertise to the care team
- Participate in quality improvement initiatives
About You
You are a well-rounded, collaborative nurse who enjoys working in primary care and building ongoing patient relationships.
Qualifications & Skills:
- Registered Nurse in good standing with the BCCNM
- Minimum 1 year of recent relevant experience or equivalent training in a community setting
- Solid understanding of: Chronic disease management, Primary care and preventative health, and Care across the lifespan (families to seniors)
- Experience with patient education, care coordination, and interdisciplinary teamwork
- Strong communication skills and ability to engage diverse patients
- Commitment to culturally safe, respectful care
- Organized, adaptable, and comfortable managing a variety of clinical needs
- A continuous improvement mindset
About Mid-Main Community Health Centre
Mid-Main is a non-profit organization providing accessible, team-based primary care in Mount Pleasant, Vancouver. Our centre brings together a medical clinic, dental clinic, and pharmacy in one location.
Our medical clinic supports approximately 30,000 patient visits annually, providing longitudinal care to a diverse community of families, individuals, and seniors. We also offer a parent outreach program supporting families with young children.

Compensation & Benefits
- $45.00 – $55.91 per hour (based on experience)
