CN Taylor Medical practice is a busy, patient-centered family medicine practice committed to providing comprehensive, compassionate, and evidence-informed care to individuals and families across the lifespan. Our clinic has a growing focus on women’s health, including menopause care, preventative health, chronic disease management, and wellness support.We are seeking a collaborative and motivated Nurse Practitioner to join our multidisciplinary team and support the continued growth of our women’s health and menopause services, enabling women to embrace this stage of their life.
Position SummaryThe Nurse Practitioner will provide comprehensive primary care with an emphasis on women’s health and menopause management. The successful candidate will work collaboratively with the physician and allied health professionals to deliver high-quality care focused on prevention, education, treatment, and long-term wellness.This role is ideal for an NP with an interest or experience in:
- Menopause and perimenopause care
- Hormone therapy management
- Preventative women’s health
- Sexual and reproductive health
- Chronic disease management
- Health promotion and patient education
Key Responsibilities
- Provide comprehensive assessments, diagnosis, treatment, and follow-up care for patients across the lifespan
- Deliver specialized menopause and women’s health care, including symptom management and hormone therapy support
- Order and interpret diagnostic tests and screening investigations
- Prescribe medications and therapies within NP scope of practice
- Provide preventative care and health education
- Manage chronic diseases and support lifestyle and wellness planning
- Maintain accurate and timely clinical documentation using EMR systems
- Collaborate effectively within an interdisciplinary care team
- Support quality improvement initiatives and evidence-informed practice
- Documents appropriately within the Electronic Medical Record and other avenues of documentation as required
- Facilitates continuity of care by working in collaboration with professional health care teams, including specialists
- Consultation with physician in accordance with the standards for physician consultation
- Maintains accurate, concise and confidential documentation
- Advocates for patients, families and caregivers when required
Hours of Work
- Mon-Friday
- Ability to either relocate or commute to Sarnia, ON
Qualifications
- Registered Nurse in the Extended Class (RN(EC)) / Nurse Practitioner registration in good standing with the provincial regulatory college
- Current BCLS certification
- Current liability protection and professional insurance
- Experience in primary care or family medicine preferred
- Interest or experience in menopause and women’s health considered a strong asset
- Strong communication, organizational, and relationship-building skills
- Ability to work independently and collaboratively in a fast-paced environment
- Experience with electronic medical records (EMR) systems preferred
- Provides leadership in the application of nursing knowledge to nursing team within the practice
- Baccalaureate in Nursing (BScN) required
- Masters Degree in Nursing (MScN or MN) required
- Member of RNAO with professional liability insurance required
- 2 years experience preferred
- Fully vaccinated for Covid-19 (will provide proof)
