The purpose of this position is to maintain or improve the health status of clients of Home Care by providing quality physiotherapy services to clients, families, and caregivers in their homes. The home care physiotherapist works independently and participates as an integral member of the Home Care team composed of nurses, home support workers, occupational therapists, social workers, and administrative assistants and promotes physiotherapy services and understanding of the profession.
Duties will include but are not limited to:
- Providing physiotherapy assessment, consultation, analysis (including diagnosis and prognosis within professional scope), and treatment, including utilization of and recommendation/ clinical prescription of specialized equipment, based on best practice evidence;
- Recording all aspects of the client’s assessment and treatment including initial assessment, client goals, treatment plans, progress notes and transfer/discharge notes;
- Participating in and/or leading case conferences and collaborating in care planning activities with other health care professionals, families and/or community partners;
- Maintaining a culture of safety within the organization and are aware of role in supporting client safety within the organization;
- Providing guidance to Physiotherapists and other healthcare professionals through clinical program leadership and/or clinical expertise;
- Developing, implementing, coordinating and leading designated client population/program/service including collecting, monitoring, evaluating and reporting data to ensure quality assurance;
- encouraging and supporting clients and their families in their efforts to take responsibility for promoting, maintaining and enhancing their health;
- leading and/or participating in the development of specific programs, relevant protocols, and policies and procedures;
- participating on committees and staff meetings. This includes work on a provincial basis;
- participating in evaluating the home care programs and services;
- performing other related duties as required.
Qualifications
Minimum Qualifications:
- The successful applicant must have successful completion of a Master’s Degree or equivalent in Physiotherapy.
- Must be eligible for licensing with the Prince Edward Island College of Physiotherapists.
- Considerable experience as a practicing Physiotherapist.
- Have a good previous work and attendance record.

- Demonstrated ability to work independently within professional expertise and apply sound clinical decision making.
- Demonstrated ability to work cooperatively and collaboratively in an interdisciplinary team environment.
- Strong communication skills, both written and verbal.
- Effective time management and organizational skills.
- Demonstrated commitment to continuous professional development.
- Proficiency working in a computerized environment.
- Ability to develop and lead training sessions for clients and staff.
- Possess a valid driver's license and have access to a reliable vehicle.
- Have or be committed to acquire training necessary to perform role.
- Demonstrated leadership, interpersonal, facilitation and collaboration skills.
- Additional relevant education and experience will be considered an asset.
- Community experience considered an asset.
