Detailed Job Description
- Review prescriptions for correct dosage, frequency, route, duration, duplication, drug interactions, contraindications, allergies and patient-specific risks before dispensing or recommending clarification.
- Dispense medicines accurately with clear labels and counsel patients on use, timing, storage, side effects, missed doses, warning signs and follow-up requirements.
- Coordinate with doctors and nurses to clarify prescriptions, suggest appropriate alternatives where permitted, support medication reconciliation and reduce avoidable medication errors.
- Maintain stock levels, expiry checks, cold-chain medicines, high-alert medicines, emergency medication availability, purchase requisitions and inventory records.
- Support medication safety audits, adverse drug reaction reporting, antibiotic stewardship, controlled medicine documentation and pharmacy SOP compliance.
- Prepare discharge medicines, outpatient medicine packs, day-care procedure medication support and patient education materials as required by the facility.
- Maintain billing coordination, regulatory registers, pharmacy records, incident notes and documentation required for audits and internal reviews.
- Follow facility rules, confidentiality expectations, local pharmacy practice requirements and employer policies for patient safety and professional conduct.
