Detailed Job Description
The Clinical Pharmacist will review medication orders for dosage, route, frequency, duplication, allergy concerns, contraindications, drug interactions, renal or hepatic dose adjustments, and antibiotic use where applicable. The role includes clarifying prescriptions with doctors, supporting medication reconciliation during admission and discharge, checking high-alert medicines, counselling patients on correct use of medicines, and helping reduce preventable medication errors. The pharmacist will also assist in monitoring stock movement, expiry control, controlled-drug handling where applicable, formulary compliance, and documentation linked to pharmacy quality standards.
The position requires a balance of technical knowledge and professional communication. The pharmacist must be able to explain medication instructions clearly to patients and families, especially for chronic diseases such as diabetes, hypertension, cardiac illness, respiratory disease, post-operative care, and long-term antibiotic or anticoagulant therapy. The role also involves collaborating with nurses on administration queries, supporting audit activity, reporting adverse drug reactions as per facility policy, and participating in internal training sessions related to medication safety.
