The Role
As a Care Manager, you will own the day-to-day care experience for a group of families. You will
coordinate home visits, support our field care professionals (CareMates), speak with families abroad,
escalate medical concerns, and make sure every parent under our care feels seen, heard, and looked
after. This is a remote role based out of Kerala, with occasional travel to family homes, partner clinics,
and our Kerala office.
What You Will Do
- Coordinate scheduled home visits between CareMates, families, and partner labs or clinics
- Build trusted relationships with parents in India and their adult children abroad through calls, WhatsApp, and visit reports
- Review CareMate visit notes and flag anything unusual
- Handle urgent situations calmly, hospital accompaniment, lab follow-ups, doctor appointments, and keep the family informed in real time
- Onboard, support, and coach new CareMates as the network grows
Requirements
- Healthcare or eldercare experience is a plus
- Experience in Care coordination, home health, or geriatric care is a plus
- Strong judgment and ability to recognize when to escalate a concern
- Excellent verbal and written communication skills
- Comfortable using WhatsApp, email, and standard office software; able to learn new tools quickly
- Fluent in English and Malayalam
- Experience interacting with patients’ families is preferred
Nice to Have
- Prior experience in geriatric care, home health, or hospital coordination
